New Patient Form Reception at the Gavin Lobo Health clinic

Free to enrol
Registering costs nothing, though a consultation fee applies each time you're seen after that. Casual, non-enrolled patients pay a higher rate
Community Services and High Use Health cards
You may pay less if you hold one of these cards, even at a practice where you aren't enrolled
Care Plus
Extra funding is available if you're managing a long-term health condition or a terminal illness
ACC-covered accidents
Injuries covered by ACC are charged at a reduced fee
Specialist referrals
Public referrals are free but may involve a wait. Private specialist care is available sooner, generally at your own cost unless you're insured
Changing your doctor
Enrol with us at any time and we'll handle the transfer of your records from your previous GP
