Methylphenidate vs Dexamphetamine: What’s the Difference?

The difference between the two stimulant families used to treat ADHD, and why we can't predict which will suit you.

If you’ve recently been diagnosed with ADHD, you may have heard names such as methylphenidate, Ritalin, Rubifen and dexamphetamine.

One of the most common questions is:

Which ADHD medication is best?

The answer is that there isn’t one medication that is best for everyone.

Two different stimulant families

Methylphenidate and dexamphetamine are both stimulant medications used in the treatment of ADHD.

Both affect brain signalling involving dopamine and noradrenaline—chemical messengers involved in attention, motivation, working memory and executive functioning.

However, they do this in somewhat different ways.

That difference matters because individual people can respond very differently to each medication.

Why can’t we predict which one will work?

Unfortunately, there isn’t currently a simple blood test, brain scan or genetic test that can reliably tell us which stimulant medication will work best for an individual patient.

Someone may experience excellent symptom control with methylphenidate and troublesome side effects with an amphetamine-based medication.

Another person may experience exactly the opposite.

That’s why ADHD treatment needs to be individualised.

What should the right medication do?

Regardless of which medication you take, successful treatment should improve everyday functioning.

You may notice:

Better concentration.
Easier task initiation.
Less distractibility.
Improved impulse control.
Better organisation.
Improved ability to complete tasks.
Less mental effort required to manage everyday responsibilities.

You should still feel like yourself.

What if the first medication doesn’t work?

It doesn’t mean ADHD medication has “failed.”

Sometimes the dose needs adjusting.

Sometimes the timing isn’t right.

Sometimes a different formulation works better.

And sometimes another medication family is a better fit.

Finding the right medication can take time.

We gradually assess both benefit and side effects rather than rushing toward a particular dose.

The goal isn’t the strongest medication or the highest dose.

It’s finding the treatment that works best for you.
