How Do I Know If My ADHD Medication Is Working?

How to tell whether your ADHD medication is actually working, beyond how you happen to feel on any one day.

One of the most common questions we hear from adults starting ADHD treatment is:

“How am I supposed to know if the medication is actually working?”

Many people expect ADHD medication to produce an obvious sensation.

Successful treatment can be much quieter than that.

Look at what you do, not just how you feel

ADHD medication is intended to improve symptoms such as inattention, impulsivity and difficulties with executive functioning.

Rather than looking for a dramatic feeling, pay attention to everyday life.

You might notice:

  • Starting tasks requires less effort.
  • You procrastinate less.
  • You’re able to stay with a task for longer.
  • You finish more of what you start.
  • You’re less distracted during conversations.
  • You interrupt people less frequently.
  • You’re managing appointments and deadlines better.
  • You feel less overwhelmed by ordinary responsibilities.
  • It’s easier to pause before reacting impulsively.

These changes can initially be easy to miss.

You may not think, “Wow, my medication is working.”

Instead, you might suddenly realise that you’ve answered your emails, put the washing away and made the appointment you’ve been postponing for three weeks.

Other people may notice first

Partners and family members sometimes recognise improvements before the person taking medication does.

They might notice you’re more present during conversations, less likely to interrupt, calmer when something goes wrong, or more likely to complete everyday jobs.

During titration, it can be helpful to ask someone you trust:

“Have you noticed anything different?”

Their observations can provide useful information.

One bad day doesn’t mean your medication has stopped working

Medication doesn’t remove ADHD completely.

Sleep, stress, illness, nutrition, hormones and the demands of a particular day can all influence concentration and executive functioning.

Everyone still has unproductive days.

Instead of judging your medication based on one particularly good or bad day, look for patterns over several days or weeks.

Keep a simple record

During titration, consider recording:

Focus: Was it easier to concentrate?

Task initiation: Was starting things easier?

Task completion: Did you finish more?

Impulsivity: Was it easier to pause before acting or speaking?

Mood: Did you feel calmer or less overwhelmed?

Duration: When did you notice the medication starting and wearing off?

Side effects: Did you experience appetite changes, headaches, anxiety or sleep problems?

This information can be extremely useful when reviewing your medication with your clinician.

The goal is not perfection

ADHD treatment isn’t about becoming endlessly productive.

The goal is to make everyday life more manageable and less exhausting.

If things that once required enormous mental effort gradually become easier, that can be one of the strongest signs that treatment is heading in the right direction.
